This form is designed for use by existing Sparrow Regional Medical Supply customers only.

The form will be sent to the location you choose and your order will be ready for pick-up in 3 hours or the next business day if the order is placed after 3:00 p.m.

U.P.S. shipping is at no charge and will take 2 days for delivery. Orders placed after 3:00pm will be shipped the following day, orders placed on the weekend will be shipped on Monday or next business day.
